The מגן אברהם ס' תקפ"א ס"ק י"ז quotes the הגה במנהגי מהרי"ק that one should begin every Selicha with:
unless the Selicha itself begins with the Name of Hashem.
There are many printers of Selichos who apparently were unaware of this מגן אברהם and printed אלוקינו ואלוקי אבותינו before Piyutim that already begin with Hashem's Name.
One example is the Piyut:
(סליחות ערב ראש השנה)
The same issue appears in some editions with the Piyutim of צום גדליה and on the fourth day of עשי"ת, where the Piyut likewise begins with Hashem's Name.
