EVEN WORSE THAN SPEAKING לשון הרע

לא תלך רכיל בעמיך לא תעמד על דם רעך

R. Chaim Volozhiner Zt"l said," לא תעמד על דם 
is written right after לא תלך רכיל  to teach us
that there are times when one is obligated to
speak  לשון הרע, otherwise, he will be עובר
on לא תעמד על דם רעך"
 
  The פתחי תשובה על או"ח ס' קנ"ו writes, that not
  speaking לשון הרע when there is a need to do
  so, is a bigger עבירה than speaking לשה"ר
